Gym H & S questionaire

Attached is a questionaire I filled in for the gym we use in bournemouth.
Thought it might be useful for people trying to find gyms orwho get given such a form

Hi,

Tried opening your images, but to small to view and quality is lost when I enlarge it. Any ideas what I should open it with?

I have contacted a friend who works for a leisure centre and he said its all down to H&S, so would be interested in seeing what you have filled in.

You need to click on the images to enlarge them.

I click and they don't enlarge. They look like unlinked thumbnails from imageshack.

They are indeed imageshack thumbnails Spud... From my images from when I scanned them in for Q and uploaded to the JJ forum. Since that's been rebooted, they're not on there, I'll have a look if they're on my old hard drive.


TADA, I've only got the rough photo version that I took at uni, not the scanned ones, but they're good enough.
